A person seeking issuance of a permit under section 13-2 shall file a written application with the clerk of the board of supervisors which
shall include:
(a)
The name and address of the applicant.
(b)
The name and address of the person sponsoring the dance, if any.
(c)
The day and hours for which the permit is desired.
(d)
The premises or portion thereof for which such permit is desired.
(e)
An estimate of the anticipated attendance.
